When getting in contact with Evolve Mental Health to make a general enquiry, the user may be requested to provide Evolve Mental Health with some personal information including; (name, email, phone number).  Once gathered, this personal information will be used by Evolve Mental Health for responding to the enquiry made and to track further correspondence.
